• Please download from the web site and complete the Game Sheet and bring along 10 copies when you register your team.
• Please download and complete one copy of the Team Discipline Log and submit it when you register your team.
• Please download and complete one copy of the Bench Staff List and submit it when you register your team.
• A Travel Permit is required by ALL out-of-district teams. Basically, if your players' books contain a district stamp other from than the EODSA, you require a Travel Permit - even if you play in the ERSL. Please FAX the Travel Permit in advance to 613-237-1890. Quebec teams: Please note that you need TWO stamps of approval on your Travel Permits.
• Registration will be held at the Jim Durrell Centre on Walkley Road (about 300 yards east of Bank Street) from 6:00 pm until 9:00 pm on Thursday May 22 (for the Girls' Icebreaker) and on May 29 (for the Boys' Icebreaker).
• At Registration, you will need to have your player cards (or books) properly completed and approved by your District Association. You will also need to show the Official OSA Team Roster for your team AND FOR THE USUAL TEAM FOR EACH GUEST PLAYER. Please note that the OSA Roster DOES NOT replace any player's missing card or book. You must have the cards or books. Although we will NOT keep a copy of the Official Roster, you will need to keep it on hand in case any player eligibility questions (etc) arise during the tournament.
