Instructions and Registration:
PLEASE NOTE:
1. Men’s and Old-Timers and Women’s are using different registration forms
2. Ontario Soccer Association regulations state:
- all players born in 1991 or before must register themselves.
- all players born in 1992 or sooner must be registered by a parent or guardian.
3. The early bird date for the 2010 season is March 31st.
To Register:
- Review the OSA Waiver and Participation Agreement (Click here)
- Review the Registration Instructions and Refund Policy (Click here)
- Print up, fill and send the REGISTRATION FORM ONLY along with a cheque made out to OISC for the full amount to OISC, 1237 Newmarket St., Ottawa, ON K1B 5N6
