2. OSA Waiver and Participation Agreement
3. Girls Competitive Registration Instructions and Refund policy

Please read through documents 2 and 3.
Please print, complete, sign and send in the Girls Competitive Registration form along with a cheque for the full amount to the address on the form.

Please note:

1. The early bird date is February 25th, 2012.
   
2. Tryouts will be starting in early March.
   
3. In order to be able to participate in tryouts players must be registered and paid in full.
   
4. Try-out times will be posted on the club website and emailed by February 21.

You will be receiving confirmation of your registration by email.
